The West Indies won the toss and elected to bat, with Australia leading the series 2-0 and eager to seal a series whitewash.
Bartlett impressed with figures of 4-17 and 4-21 during the ODI series win against the West Indies and he's now been given the nod for his T20 debut.
The 25-year-old replaces Josh Hazlewood, who has been rested ahead of the tour of NZ.
In Australia's other change Aaron Hardie comes in for fellow West Aussie Marcus Stoinis.Â
Rising star Fraser-McGurk, who had been brought into the squad as cover, has been named 12th man.
Mitch Marsh will again skipper the side.
"It's pretty cool," Marsh said of captaining the national side on his home turf.
"We're looking forward to hopefully closing out this series."
The West Indies rested opener Brandon King and Shai Hope and brought in Kyle Mayers and Roston Chase.
